Your Patrick is still alive in 1910 Concord and has his youngest child Anna/Annie living with him. He's shown as a widower. I noticed that the 1920 census has a Patrick Conway age 76 living in a soldiers home in Tilton, Belknap Co (not far from Concord). It's possible this was your Patrick. You might want to look into whether he had a Civil War pension. Those pension files can have lots of interesting information. I have ordered a few in the past. I think you can order online with form NATF 86 from this link

His dob, place of birth, town of residence after the war, and later possible residence in the soldiers home, his wife's name and names of children should be enough for the Nat'l archives to locate the correct Patrick if indeed your Patrick did serve.

If his wife and children were all dead before 1920 (not sure where the children ended up) it's possible he was buried right on the grounds of the soldiers home. You could check it out.

If this is not your Patrick you might want to check out cemeteries in Concord to see if he is buried there. His headstone should at least give you a year of death and then you can order a death cert.

the 1900 census has additional child Anna/Annie born 1882
